[ES]Las personas con Trastornos Mentales Graves presentan amplias necesidades que el tratamiento convencional se ve limitado para resolver satisfactoriamente. El modelo de Tratamiento Asertivo Comunitario surge ante un intento de mejorar la atención sanitaria en la comunidad a personas con TMG, modificándola hacia servicios comunitarios de apoyo y seguimiento y definiendo una serie de características que permitieran que el paciente redujese los ingresos hospitalarios y no perdiera el contacto con los servicios comunitarios. El objetivo de esta revisión es identificar la eficacia del TAC en la prestación de cuidados que garanticen una atención comunitaria e integral, por lo que se lleva a cabo una revisión de la literatura científica en múltiples bases de datos, obteniendo un total de 23 documentos y evidenciándose la eficacia del programa en los reingresos hospitalarios, satisfacción, calidad de vida y sintomatología de los pacientes.
[EN] People with serious mental disorders present a wide range of needs that conventional treatment is limited to resolve satisfactorily. The Assertive Community Treatment appears to improve the sanitary attention in the community to persons with SMD, modifying it to community services and defining characteristics that allow the patient would reduce hospital admissions and not lose the contact with community services. The objective of this review is to identify the efficacy of TAC in providing care to ensure a community and comprehensive care. A review of the scientific literature in multiple databases is done, obtaining a total of 23 documents and demonstrating the effectiveness of the program in hospital readmissions, satisfaction, quality of life and symptoms of patients.
