[EN] Peripheral arterial disease is one of cardiovascular diseases, which are very important in the fields of healthcare. By-pass surgery in lower limbs, is a surgical treatment that patients with this condition could need to improve symptoms caused by decreased peripheral circulation. Based on available scientific evidence, we selected factors that should be included in care and assistance during this stage of disease, and we developed a Standardized Care Plan for patients with peripheral arterial disease treated with by-pass surgery. This Standardized Care Plan includes as main diagnoses: ineffective peripheral tissue perfusion, control risk of impaired skin integrity, activity intolerance and ineffective management of their own health. To manage these diagnoses, we establish expected outcomes of health and interventions to achieve them.
