[EN] Colorectal cancer is the most common incident in Spain, considering both sexes. The treatment in the first phase consists in a bowel resection of the affected area, joining, by means of surgery, the healthy ends of the bowel, known as intestinal anastomosis. When it is not possible to perform this, a natural stoma is opened in the body with a portion of the bowel in the abdomen. . Therefore, this assignment is aimed at describing an Individualized Healthcare Plan for patients with the above-mentioned disease in order to get their independence in their stoma cares, focusing on their need of elimination which is altered. It is based on an assessment according to Virginia Henderson, identifying the Nursing Diagnosis (NANDA), carrying out the Nursing Interventions Classification (NIC) and the Nursing Outcomes Classification (NOC) showing the work of the Health Education in order to achieve autonomy and the acceptance of the stoma.
